What Must Be Reported
Child Abuse
ORS 419B.010 (Duty of officials to report child abuse): Any public or private official having reasonable cause to believe that any child with whom they come in contact has suffered abuse or that any person with whom they come in contact has abused a child will immediately report or cause a report to be made as required in ORS 419B.015 (Report form and content).
ORS 419B.015 (Report form and content): Anyone making a report of child abuse will make an oral report by telephone or otherwise to the local office of the Department of Human Services, to the designee of the department or to a law enforcement agency within the county where the person making the report is located at the time of the contact. - The names & addresses of the child & the child's parents or anyone else responsible for the child's care of the child
- The child’s age,
- The nature and extent of the abuse, including any evidence of previous abuse,
- The explanation given for the abuse and
- Any other information that the person making the report believes might be helpful in establishing the cause of the abuse and the identity of the perpetrator

Elder Abuse
ORS 124.060 (Duty of officials to report elder abuse): Any public or private official having reasonable cause to believe that any person 65 years of age or older with whom they come in contact has suffered abuse, or that any person with whom they comes in contact has abused a person 65 years of age or older, shall report or cause a report to be made in the manner required in ORS 124.065 (Method of reporting).
ORS 124.065 (Method of Reporting): When reporting required under ORS 124.060 an oral report will be immediately made by telephone or otherwise to the local office of the Department of Human Services or to a law enforcement agency within the county where the person making the report is at the time of contact. - The names and addresses of any persons responsible for their care

- The name, age and present location of the allegedly abused adult;
- The names and addresses of persons responsible for the adult’s care;
- The nature and extent of the alleged abuse, including any evidence of previous abuse;
- Any information that led the person making the report to suspect that abuse has occurred plus any other information that the person believes might be helpful in establishing the cause of the abuse and the identity of the perpetrator; and
- The date of the incident
