Device Checklist
- Check the power indicator on the instrument to ensure it's on.
- "Ready to Start” should be displayed.
- Check date & time are correct.
- If incorrect note the correct time and date on the report and contact OSP Implied Consent.
- Print Intoxilyzer operator checklist (F2 key)
- Fill out the top section with the:
- Test date, driver’s name, arresting officer's name & department
- Requires the name of the officer observing the suspect
- The observer and/or operator don't have to be the arresting officer.
- Check off the rest of the steps as they're completed
- Complete the Operator’s Checklist with the operator's
- Name, department, signature, permit #
Device Setup
- Press the “Start Test” button.
- 'Jail release (Y/N)?' Is displayed.
- Press “Enter”, the default is “N”.
- “Scan Operator Permit or Press Enter” is displayed
- Hold permit flat perpendicular to instrument's face directly above the keyboard until the bar code on the permit is read
- Scanning successful: a tone is emitted & “Enter PIN #” will be displayed
- Scanning fails: the instruments enter manual mode. See below.
- Pressing the “Enter” key puts instrument in manual mode & prompts for permit information.
- “Enter Operator PIN” is displayed on successful permit entry
- Enter the four digit PIN # and press the “Enter” key.
- The operator has three attempts to enter the correct PIN before the test is aborted
- “Scan Subject License or Press Enter” is displayed.
- The operator may scan the driver’s license or press “Enter” to manually enter the driver’s information
- “Review Data (Y/N)?” Is displayed
- If “Y” is selected, subject Name, DOB, ID#, and State of Issue is displayed
- Subject info may be edited if needed
- Each entry must be accepted by pressing the “Enter“ key.
Device Self-Test
- “Ready in XX:XX” (usually about 30 seconds or less)
- “Air Blank” is displayed & an internal purge pump operates for approx 20 secs
- a self-diagnostics check is performed:
- "Diagnostics Pass” displayed, or
- “Diagnostic Fail” displayed & “Invalid Test – Diagnostic Fail” printed on the test report.
- The above steps are performed again
- “Air Blank” is displayed & an internal purge pump operates for approx 20 secs
- On error “Ambient Failure” displayed and “*Invalid Test – Check Ambient Conditions” printed the test report
- Breath sampling period: A progress bar is briefly displayed as a series of rectangular characters
- “Please blow into mouthpiece to activate tone” is displayed in the display's top line
- The device is ready to perform a breath test
Breath Test
- Read the following statement to the subject (located on the front of the instrument):
- I WANT YOU TO TAKE A DEEP BREATH AND BLOW INTO THE INSTRUMENT WITH ONE LONG, CONTINUOUS BREATH. BLOW WITH SUFFICIENT FORCE TO INITIATE AND MAINTAIN THE AUDIBLE TONE. CONTINUE BLOWING UNTIL I TELL YOU TO STOP. DO YOU UNDERSTAND THESE INSTRUCTIONS?
- If necessary, re-instruct subject on giving a proper breath sample & indicate that failure to comply will result in a refusal
- Have the subject provide a breath sample
- Proper sample delivery is indicated by a continuous audible tone & “Please Blow” displayed.
- Subject not cooperating with the breath test. Re-instruct. If determines the subject is refusing:
- Abort test using “A” key (only during the 3 minute breath sample period).
- Maintain Observation of subject
- Read NORAC section IB
- Ask the question after IB, “Will you provide physical cooperation and submit to a breath test”
- Re-start the test
- Suspend for refusal if subject does not complete the test by pressing the “R” key (only during the 3 minute breath sample period).
- Articulate the refusal.
- What did the subject say and do that indicated refusal?
- What other observations are there regarding the subject’s ability to provide a breath sample?
- A “period” displayed after “Please blow” indicates adequate sample collection
- Instruct the subject to stop blowing.
- “Air Blank” is displayed & an internal purge pump operates for approx 20 secs
- "Please Wait XX:XX remaining” is displayed as a two minute observation time counts down
- Observe the subject ensuring they don't take anything by mouth, vomit, or regurgitate liquid from the stomach into the mouth
- Breath sampling period: A progress bar is briefly displayed as a series of rectangular characters
- “Please blow into mouthpiece to activate tone” is displayed the int display's top line
- The device is ready to perform a breath test
- Perform steps 1 through 4 above again
Post Test
- “Add Comments (Y/N)?” Is displayed. Enter:
- “N” - the device prints test reports.
- “Y” - The device displays three (3) comment messages, starting with “Comment Line 1”
- Three (3) copies of the complete test report are printed
- “Reprint Test (Y/N)” is displayed
- Complete operator’s checklist with the operator’s name, department, signature and operator’s permit #
- Breath test failure
- Criteria
- BAC 0.8% or more if not driving a commercial motor vehicle
- BAC 0.4% or more if driving a commercial vehicle
- Any amount for person under 21
- On the ICCR 'Test Failure Section'
- Fill in Instrument serial number
- Found on instrument's side, or
- The at the bottom of the instrument checklist
- Tick one of the suspension period boxes
- Tick the DMV hearing conducted in person box
- Fill in: Signature, Agency, Telephone #, Print Name & Agency ID, DPSST #
- ICCR temporary permit form
- If seizing ODL and driver eligible for temp permit
- Tick 'Yes' box next to 'Eligible for temporary permit'
- Fill in licence type, restrictions, & endorsement from ODL
- Fill in effective from & to date
- Fill in from D07
- Have driver sign or know to sign permit to make it valid
- If driver is not eligible
- Tick the 'No' box next to 'Eligible for temporary permit'
- Breath test Refusal
- On the ICCR 'Test Refusal' Section'
- Tick 'You refused to submit to a breath test'
- Tick the one or three year suspension box
- Tick the DMV hearing conducted in person box
- Fill in: Signature, Agency, Telephone #, Print Name & Agency ID, DPSST #
- ICCR temporary permit form
- If seizing ODL and driver eligible for temp permit
- Tick 'Yes' box next to 'Eligible for temporary permit'
- Fill in licence type, restrictions, & endorsement from ODL
- Fill in effective from & to date
- Fill in from D07
- Have driver sign or know to sign permit to make it valid
- If driver is not eligible
- Tick the 'No' box next to 'Eligible for temporary permit'
- Breath test pass
- Tick the ICCR 'Driver did not fail test nor refuse to submit…' box
- DO NOT submit any forms to DMV. Keep for own records
Test Reports
- On test completion, the device prints out 3 copies of a test report
- All considered originals and must be kept
- Two types of test reports: Complete and incomplete
- Incomplete test reports are different from complete test reports as follows:
- Test result won't have any numerical information, a refusal or an interfering substance detected statement.
- A message is printed after the “Additional Comments” section followed by additional info
- A “Suggested Corrective Action” statement may be printed
- Strict adherence to suggested correction actions is neither required nor necessary to correct an exception condition.
- Test successful
- There will two BAC numbers provided on the test report
- Note: different BAC numbers on a single report don't indicate a “rising” or “falling” BAC
- Test refused
- Operator terminated test sequence due to subject's refusal to continue
- Test is complete and should not be restarted (unless operator error)
- Generated by pressing "R" key or "Start/Stop" button during 3 minute breath sampling
- Print and submit into evidence
- Interfering substance detected
- A substance other than alcohol detected in the breath sample
- The test is complete & should not be restarted.
- A test report indicating “Subject Test Refused” is not required to document a refusal
- Corrective actions:
- Discontinue the Implied Consent process.
- Ask the subject about diabetes, fasting, & solvent exposure.
- If diabetic, consider EMS assistance.
- Consider requesting a voluntary blood sample from the subject or obtain a warrant.
- Any observations made during the testing sequence may be recorded in the comments section.
- Ambient fail
- Alcohol, or a contaminant, introduced into the sample chamber from the surrounding room air during an “Air Blank”.
- Ambient Fail” displayed & “*Invalid Test – Check Ambient Conditions” printed
- Instrument sucked in alcohol or a contaminant
- The test is not complete & can be restarted
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Consider removing any contaminants from the area (including the subject, if necessary).
- Use a new mouthpiece and restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- Deficient sample
- The subject did not blow long enough with sufficient force to properly provide a sample during the breath sample period
- "Deficient Sample” displayed and “*Invalid Test – Deficient Sample” printed
- The test is not complete and can be restarted
- unless the operator determines that the subject is physically incapable of providing a proper sample).
- “F6” auto populate – If test needs to be restarted
- Subject physically incapable of providing breath sample:
- Articulate the reason.
- If the subject is able to provide the first sample then there is no reason why they can not provide a second, this would be a refusal.
- here is not a BAC% result on a “Deficient Sample”
- Corrective actions:
- Consider re-instructing the subject on how to provide a proper breath sample.
- If continuing with the testing process, use a new mouthpiece and restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- Improper sample
- The subject blows into the mouthpiece at an improper time during the testing sequence
- “Improper Sample” displayed and “*Invalid Test – Improper Sample” printed
- Operator must maintain control of the breath tube
- The test is not complete and can be restarted
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Consider re-instructing the subject on how to provide a proper breath sample.
- Use a new mouthpiece and restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- No sample given
- The subject didn't blow hard enough into the device to activate the audible tone during the breath sampling period.
- “No Sample Given” displayed & “*Invalid Test – No Sample Given” is printed
- The test is not complete and can be restarted
- Corrective actions:
- Consider re-instructing the subject on providing a proper breath sample.
- If continuing testing process, restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- Purge fail
- Device unable to purge the sample cell after running a breath test.
- May be due to the purge pump not operating or breath path being obstructed.
- “Purge Fail” displayed & “*Invalid Test – Purge Fail” printed
- The test is not complete and can be restarted
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Remove mouthpiece from the breath tube and check for obstructions
- Use a new mouthpiece and restart the test
- If an additional exception message is given, consider placing the instrument out of service
- Leave the instrument power on and consider locating an alternative testing location
- Record observations made during test sequence in the comments section
- Range exceeded
- The test result was higher than 0.60 %BAC
- “Range Exceeded” displayed and “*Invalid Sample – Range Exceeded” printed
- May be due to a high concentration of residual alcohol in the subject’s mouth or upper respiratory tract from recent consumption of alcohol or having vomited or regurgitated liquid from the stomach into the mouth
- The test is not complete and can be restarted.
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Consider checking the subject’s mouth.
- Conduct another pre-test wait/observation period of at least 15 minutes.
- Use a new mouthpiece and restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- Residual alcohol present
- Residual alcohol detected in the subject's mouth or upper respiratory tract.
- Possible recent consumption of alcohol, vomiting or liquid regurgitation
- ⁃ “Residual Alcohol Present” displayed & “*Invalid Sample – Residual Alcohol Present” printed
- The test is not complete & can be restarted. “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Consider checking subject’s mouth.
- Conduct another pre-test wait/observation period of at least 15 minutes.
- Use a new mouthpiece and restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- RFI detected
- A radio or cellular phone transmission is detected near the device
- “RFI Detected” displayed & “*Invalid Test – RFI Detected” printed
- The test is not complete and can be restarted
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Remove interference source from device's vicinity ⁃ Use a new mouthpiece and restart the test.
- If an additional exception message is given, consider placing the instrument out of service
- Leave the instrument power on and consider locating an alternative testing location.
- Record observations made during test sequence in the comments section
- Sample correlation failure
- the subject did not provide two similar valid breath samples
- May be result of improper breath sample delivery or residual alcohol in the subject's upper respiratory tract
- “Sample Corr. Fail” displayed & “*Invalid Test – Subject Sample Correlation Failure” printed.
- The test is not complete and can be restarted
- “F6” auto populate – If test needs to be restarted
- Corrective actions:
- Consider checking the subject’s mouth.
- Conduct another pre-test wait/observation period of at least 15 minutes.
- Consider re-instructing the subject on how to provide a proper breath sample.
- Use a new mouthpiece & restart the test.
- Continue the test until the subject properly submits to or refuses the test.
- Record observations made during test sequence in the comments section
- Sequence aborted
- The operator aborted the testing sequence.
- “Sequence Aborted” displayed & “*Invalid Test – Sequence Aborted by Operator” printed
- The test is not complete and can be restarted.
- “F6” auto populate – If test needs to be restarted
- F6 only works after login
- Press F6 when the instrument asks to scan driver’s license
- Corrective actions:
- If the test was terminated in error, it may be restarted
- Record observations made during test sequence in the comments section
- Test Reports can be re-printed from the device by the operator at anytime up until the instrument’s memory is downloaded by the Department of State Police

- To Reprint a report
- From the “Ready to Start” message, press “F4”
- “Scan Operator Permit or Press Enter” is displayed
- Scanpermit or press “Enter” and type in permit#.
- “Enter Operator PIN” is displayed
- Enter personal PIN#
- On valid entry, the device will display all test reports performed by the operator still stored in the instruments internal memory
- Tests are sorted by test date, test time, and subject name.
- Use the “Page Up” and “Page Down” keys to select a report
- Press “Enter” key to initiate print the selected report
- Only Test Reports generated by the operator will be available to the operator for reprinting.
- Reprints are identical Test Reports to the originals
- Enter the desired number of reprints.
- If the test report info isn't displayed, the report may no longer be available on the device.
- Additional copies can be requested from the Oregon State Police Implied Consent Program.
Device Error Messages
- Tank Pressure Low Change Standard Soon
- If the instrument detects that the external ethanol breath standard pressure is less than approximately 75 PSI.
- Continue with the testing sequence and
- nNotify the agency responsible for hosting the instrument of the warning message.
- No Permit Found
- The instrument is unable to verify the permit number entered by the operator,
- The operator does not have a valid permit number,
- The permit number has not been entered in the instrument’s internal database, or
- The operator is using an incorrect permit number.
- Incorrect PIN
- The instrument is unable to verify the PIN entered by the operator.
- An operator is given three attempts to correctly enter their PIN before the instrument aborts the testing sequence.
- Permit Expired
- The operator’s permit has expired.
- The operator should contact the Oregon State Police Implied Consent Unit.
- Clearance Exceeded
- The operator has attempted to perform an operation with the instrument that they have not been given clearance for.
- The operator should contact Oregon State Police to arrange for a Technician to perform the attempted operation or to service the instrument.
Putting Device Out of Service
- Determine exactly what the exception message being reported by the instrument is.
- This information is required to report condition to the OSP regional dispatch centre
- Perform the suggested correction action that is provided with the exception message.
- The dispatcher will ask if the corrective action was performed prior to placing the instrument out-of-service.
- Contact the OSP regional dispatch centre and provide notification of the exception:
- OSP Regional Dispatch Cente: 1-800-292-6517
- Leave the instrument ON
- Place an Out-of-Service notification placard on the instrument
- Don't remove an out-of-service placard unless authorised to do so by an OSP trained technician
- Placard located on the back of the laminated “Local DRE” list.
- Notify the host agency and/or dispatch
- Locate an alternative testing location and restart the testing sequence.
